A purchase of $450 or greater made in a single transaction (single receipt) between 01.05.2022 - 02.28.2022 with a new or existing CFNA credit card is eligible for the $66 Prepaid Debit Card. A purchase of $150.00 - $299.99 with a CFNA credit card is eligible for the $22 Prepaid Debit Card. A purchase of $300.00 - $449.99 with a CFNA credit card is eligible for the $44 Prepaid Debit Card. The online submission must be completed by 03.15.2022. Submissions after this date are not eligible for the Prepaid Debit Card. All submissions are processed through the CFNARebateCenter.com website.
